Health Dimensions Group (HDG), a leading post-acute long-term care, and senior living consulting and management firm, is proud to announce a management agreement with KC Companies, Inc., owner and operator, of six Golden Horizons care communities located in Iowa and Minnesota.
HDG will provide management services to the following care communities: Golden Horizons, Aitkin; Golden Horizons, Crosslake; Golden Horizons, Ida Grove, Iowa; Park Lake Estates, Preston, Minn; Golden Horizons, Sandstone; and Golden Horizons, Worthington.
As management partner, HDG will continue to be guided by Golden Horizons’ philosophy of providing care to residents with dignity, privacy, individuality, and independence. Each care community combines independence for an active and social lifestyle with personal care in a warm, noble setting. The organization provides several living options including assisted living, memory care, respite care, and hospice care. Golden Horizons works with residents and family members to design a lifestyle choice that ensures long-term value, resident satisfaction, and peace of mind. Each care community provides a vast array of medical capabilities to ensure each resident’s specific needs are met.
HDG provides management and oversight for 39 senior care communities in seven states, serving more than 2,000 residents. In Minnesota, HDG manages the Boundary Waters Care Center in Ely; Chris Jensen Health and Rehabilitation Center, Duluth; Frazee Care Center, Frazee Assisted Living and Frazee Senior Villa in Frazee; Highland Chateau and Rehabilitation Center in St. Paul, Traverse Care Center and Prairieview Place in Wheaton; and Vergas Assisted Living in Vergas. In Iowa, HDG manages Lutheran Living in Muscatine; Rowley Masonic Community in Perry; and, Touchstone Healthcare Community in Sioux City.
